Reliant Earns National Recognition

It is with great pride and excitement that we announce that the Centers for Disease Control and Prevention (CDC) have recognized Reliant Medical Group as a Million Hearts 2015 Hypertension Control Champion. The award is given to organizations for exemplary performance in helping patients control their blood pressure and in preventing heart attacks and stroke.

Reliant is one of only 18 medical providers and health systems throughout the country recognized with this prestigious award!  In addition, we are the only “Champion” in Central Massachusetts. Each Hypertension Control Champion is carefully selected by a panel of CDC Judges after rigorous review.

Based on actual patient outcomes, the Million Hearts Hypertension Control Challenge identifies practices, clinicians, and health systems that have worked to achieve hypertension control rates at or above 70% with their patients through innovations in health information technology and electronic health records, patient communication, and health care team approaches. The knowledge uncovered in the competition helps the CDC promote more successful ways of achieving better blood pressure control. Million Hearts is a nationwide initiative focused on preventing one million heart attacks and strokes by the year 2017, hypertension control is a critical step in realizing that goal.

    congratulations to Reliant Medical
    group for promoting awareness and treating patients with hypertension, heart
    disease and stroke!
    I do not have this, thank God, but my Dad has a great team at reliant medical group since his first heart attack several years go!
    This disease if the number 1 silent death of men and women of all different ages and ethnic backrounds! And many women have differen symptoms that arise differently than men
