Some patients in our Geriatric Primary Care program receive in-home care from our partner, Landmark Health.  Landmark is a mobile medical group that specializes in caring for older adults in the comfort of their homes. Landmark updates your Reliant Geriatric Primary Care team after each visit and as often as necessary to meet your healthcare needs.

Patients are notified about whether or not they will receive care from Landmark during their enrollment in the Geriatric Care Program. Someone from Landmark will call patients to set up a first in-home visit.
