
Reliant Medical Group and Fallon Health have joined together to form a group called the Fallon Health and Reliant MassHealth Patient and Family Advisory Council (PFAC). The PFAC gives patients a chance to provide feedback on how we can improve your experience at Reliant Medical Group and Fallon365 Care.
Why does Reliant Medical Group have a Patient and Family Advisory Council?
The way our patients experience our health care system is of paramount importance at Reliant Medical Group. We recognize that our patients are the best resource we have to understand where and how we can make improvements in the design and delivery of our healthcare services.
What are the responsibilities of the PFAC members?
PFAC members act as advisors. As an advisor on our small group council, you’ll share your perspectives and ideas on how we can improve your experience as both the patient and as a member of Fallon 365 Care. The council plays a key role in helping both Fallon Health and Reliant Medical Group understand what we do well and where we can do better in supporting you and your family.
What is the time commitment for the PFAC?
Being part of the PFAC means that you’ll be asked to attend at least two meetings a year. These meetings will be “virtual,” so you can join on a computer or smartphone, or dial in by phone. Interpreter services and translated materials can be made available upon request. Each meeting will last up to 90 minutes.
Members are asked to commit to serving on the Council for a minimum of one year and up to three years commitment to serve for a minimum of one years.
Who should apply for the PFAC?
Any Reliant Medical Group patient 18+ with Fallon365 Care insurance can apply for membership to this PFAC. It’s very important for the group to represent the diverse patient population we serve, so we welcome applicants from different age groups, races, ethnicities, and sexual orientation. We need a diverse PFAC in order build inclusive services that respond to differing needs. We also welcome parents of children who receive their care at Reliant so that we can ensure the pediatric perspective is representative.

What Happens After I apply to the PFAC?
Applicants will be contacted on a first-come, first-served basis, and asked to participate in a brief interview. There are a limited number of spots in the PFAC. So, if you don’t hear from us, we will keep your application on file for future openings.
