Reliant Medical Group is participating in the Centers for Medicare and Medicaid Services’s ACO REACH Model. This initiative, focusing on providing care coordination for our Medicare beneficiaries, is just one of the many ways our Reliant providers care for our patients.

Nursing Homes

Generally, Medicare only covers care in a skilled nursing home if you stay in the hospital for at least three days before your admission. As a REACH ACO, Medicare allows our doctors to admit you to a nursing home any time you need skilled nursing care, even if you have not had a 3-day hospital stay.
