More than 600 primary care providers, advanced practitioners, specialists, and other caregivers are now on your healthcare team, all working to provide you with the highest quality of care possible.

You are an Important Team Member

In order to provide you with the best care possible, we ask that you maintain open communication with your healthcare team, keep scheduled appointments, and partner with us through shared decision-making regarding your health. We also hope you take advantage of MyChart, our online portal. MyChart allows you to schedule appointments, get prescription refills, view your medical record, have a video medical visit, or exchange messages with your healthcare team. Sign up for a MyChart account online or in-person at any Reliant office.

Patient Resources

If you are experiencing a health problem, please call your provider’s office to schedule an appointment. If you don’t have this number please visit our locations page for a complete list of site locations and telephone numbers. You may also call our main number at (800) 283-2556.

For your first appointment, we suggest you arrive 15 minutes prior to your scheduled visit in order to allow time to complete any necessary paperwork.

Please keep the following in mind when visiting:
• If you are a new patient, please contact your former medical provider and request that your health records be sent to your new Reliant Medical Group provider. This can take two to four weeks. Please also make sure your insurance company knows that you have chosen a new primary care provider.
• Remember to bring your insurance cards and a form of photo identification.
• Co-pays are expected at check-in.
• Cash, personal checks, Visa® and MasterCard® are accepted.
• Bring a list of medications you are taking with dosage requirements.
• If you cannot keep your scheduled appointment, please contact your provider’s office at least 24 hours in advance so that we may accommodate other patients.
