Integrated Behavioral Health Support
This service for Pride+ Care members integrates your mental health with your physical needs. Services include group therapy, surgical support, and short-term therapeutic support.
Gender-Affirming Care
Our skilled clinicians provide respectful and personalized gender-affirming hormone therapy, along with navigation support for gender-affirming surgery.
Care Coordination
Wraparound support staff help you navigate the complexities of healthcare, from understanding your insurance coverage to connecting you to LGBTQIA+-specific community resources.

Whether it’s beginning PrEP medication or counseling for safer sex practices, our Pride+ Care providers are here to support your sexual health needs.

For New Patients
If you are new to Reliant Medical Group, explore our providers. Once you have chosen a provider, call Reliant Medical Group’s Registration team Monday through Friday, 8:00 AM to 6:00 PM at 508-425-5857.

For Current Patients
If you are an existing Reliant Medical Group patient who would like to join the Pride+ Care program to access additional services and/or switch to a Pride+ Care provider, call your primary care provider’s office and request a referral to the Pride+ Care program or email our Pride+ Program Manager.

Any adult (18 years and older) who identifies within the LGBTQIA+ community, lives in Massachusetts, and is looking for a high-quality, inclusive, and supportive primary care provider.
Appointment availability varies by provider and location. Please contact us for current scheduling information.
Many Pride+ Care services are covered by your insurance as part of routine primary care or mental health benefits when provided in-network. Coverage for specific services may vary by plan.
We accept most health insurances! You can check if we accept your insurance. We do accept certain MassHealth and Medicare plans. Please check our full list or call 1-877-221-8500 with insurance questions.
Our Pride+ Care providers are located at four Reliant Medical Group Sites: Auburn, Worcester – Neponset Street, Holden, and Leominster. Patients can also see our Pride+ Care providers through virtual video visits. Reliant Medical Group uses both in-house and external Quest Diagnostics locations for lab testing. Patients utilizing telehealth services must be physically located in Massachusetts during the appointment
We have strong partnerships with local healthcare organizations that have physician experts in gender-affirming surgeries, such as those at UMass Memorial Health. Our Pride+ Care primary care providers can counsel you on general gender-affirming surgical options, write letters of support, and work with our care facilitator to identify the best gender-affirming surgeon to meet your needs.
Absolutely! Reliant Medical Group can offer our patients outstanding care in more than 30 specialties, which are located right within our practice. Our Pride+ Care PCPs may recommend Reliant Medical Group specialists or other appropriate specialists based on your clinical needs and insurance coverage.
