School Forms and Letters Available in MyChart

Schools often request a letter or form before children can start classes or join sports teams. Some children also need a letter that explains a health condition they may have or to allow them to receive medications while at school.

School forms and letters are found in the Letters section of MyChart. You can print or download a copy anytime!

You can also request a copy of a school/sports/camp letter from your child’s primary care provider’s office. Please allow three to five business days to process this request and know that your child’s physical exam must be completed within the last 12 months.

Some of our locations have extended hours on evenings and weekends (by appointment only), including Auburn, Leominster, Shrewsbury, Southborough, Westborough, and Worcester.

Our walk-in care service, ReadyMED, also offers evening and weekend hours.

Virtual ReadyMED can see patients age 4+ via video visits from 7am-11pm/7 days a week. This service is provided via our online patient portal, MyChart.

Plus you can speak to our on-call pediatric nurses and physicians for medical advice.  At Reliant Medical Group, we make it easy to give your children the care they need.
