For the fifth year in a row, Reliant Medical Group has received three prominent awards from the American Heart Association and American Medical Association. These outpatient program achievement awards recognize our commitment to reducing the risk of heart disease and stroke by improving blood pressure management, high cholesterol levels, and Type 2 diabetes care. The 2024 awards are:
Gold+ level recognition as part of the 2024 Target: BPTM program from the American Heart Association and American Medical Association. This award reflects our commitment to helping improve blood pressure (BP) control rates among our patients. To receive the prestigious Gold+ level recognition, practices must demonstrate a commitment to measurement accuracy and 70% of our affected adult patients must have their blood pressure well-controlled.
Gold-level recognition from the AHA’s 2024 Check. Change. Control. CholesterolTM program. This important program recognizes healthcare systems like ours for improving the awareness, detection, and management of high cholesterol and reducing the risk of heart disease and stroke through improved cholesterol management.
Reliant has also received the American Heart Association’s 2024 Target: Type 2 DiabetesSM Gold Award for its commitment to addressing the relationship between diabetes and the risk of heart disease and stroke. The Gold award recognizes practices that have met specified Gold-level thresholds for certain clinical measures related to diabetes control and cardiovascular risk factors, hypertension and high cholesterol.
