We are thrilled to share the wonderful news that Reliant Medical Group has received prestigious recognition from the American Heart Association (AHA) and the American Medical Association (AMA). These outpatient program achievement awards recognize our commitment to reducing the risk of heart disease and stroke by improving the management of Type 2 diabetes and blood pressure.
Each adult primary care practice received Gold+ level recognition from the AHA and AMA as part of the 2025 Target: BP™ Recognition Program for its commitment to improving blood pressure control rates among our patients. Gold+ level recognition indicates that we achieved a blood pressure control rate of 70% or greater throughout our adult patient population. The initiative aims to support the improvement of blood pressure control rates through the use of evidence-based protocols, tools, and resources, and recognizes organizations that prioritize hypertension control.
All Reliant Medical Group adult primary care practices also received Gold level recognition from the AHA as part of the 2025 Target: Type 2 Diabetes™ Program for showing impressive HbA1c control in patients with diabetes and 70% or more adherence to a focused measure of clinical quality related to diabetes control and CVD risk factors, hypertension, and high cholesterol.
We should be extremely proud of our practices and our commitment to improving patient health outcomes. This recognition serves as an encouraging reminder that we can make a significant impact in reducing health disparities related to blood pressure and glycemic control.
