Does Your Child Need a Doctor’s Letter? Check MyChart first!

This time of year, parents often need a doctor’s letter to show that their child has had a physical exam. Schools often request this before children can start classes or join sports teams. Some children also need a letter that explains a health condition they may have or to allow them to receive medications while at school.

School forms and letters are found in the Letters section of MyChart. You can print or download a copy anytime!

Don’t have MyChart? Sign up online here or in person at any Reliant office.

You can also request a copy of a school/sports/camp letter from your child’s primary care provider’s office. Please allow three to five business days to process this request and know that your child’s physical exam must be completed within the last 12 months.
