Reliant Receives Three Prominent Clinical Awards

For the fourth year in a row Reliant Medical Group has received three prominent awards from the American Heart Association (AHA) and American Medical Association (AMA)! These outpatient program achievement awards recognize our commitment to reducing the risk of heart disease and stroke by improving high cholesterol, Type 2 diabetes and blood pressure management.

Gold-level recognition from the AHA’s 2023 Check. Change. Control. CholesterolTM program. This important program recognizes healthcare systems like ours for improving quality of care through awareness, detection, and management of high cholesterol.

We have also received Gold-level recognition as part of the 2023 Target: BPTM Recognition Program from the American Heart Association (AHA) and American Medical Association (AMA) for our commitment to helping improve blood pressure (BP) control rates among our patients. To receive gold-level recognition, 70% or more of our adult patients must have their blood pressure well controlled.

Reliant has also received the 2023 Target: Type 2 DiabetesTM Gold Award from the AHA for showing impressive HbA1c control in patients with diabetes and 70% or more adherence to a focused measure of clinical quality related to diabetes control and CVD risk factors, hypertension and high cholesterol.

This recognition we have received reflects the incredible amount of hard work and dedication our teams put into their work each day on behalf of our patients.
