Reliant Medical Group is participating in the Centers for Medicare and Medicaid Services’s Next Generation Accountable Care Organization Model. This initiative, focusing on providing care coordination for our Medicare beneficiaries, is just one of the many ways our Reliant providers care for our patients. In fact, more than 25% of people with Original Medicare already go to doctors and hospitals that have joined an ACO.

Nursing Homes

Generally, Medicare only covers care in a skilled nursing home if you stay in the hospital for at least three days before your admission. As part of our ACO, Medicare allows our doctors to admit you to a nursing home any time you need skilled nursing care, even if you have not had a 3-day hospital stay. Home Visits After Hospital Stays

Medicare lets Reliant send nurses and other staff to your home up to nine times after you’re discharged from a hospital. They will help you follow the discharge plan your doctor writes.
