Highly specialized care when you need it most.

At Reliant, our goal is to provide patients with the right care, at the right time and in the right place. That’s why we developed a specialized Geriatric Primary Care program.

Our Geriatrics Primary Care team is made up of health care providers specializing in the care of people ages 75 and older. The team consists of physicians, nurse practitioners, physician assistants, nurses and social workers, all working together to care for smaller groups of older adults. They can spend more time with patients than a traditional primary care provider can, and see patients more often, if that is what is needed.

Who can join the program?

This program is being offered to eligible patients aged 75+, based on their healthcare needs, who would benefit from the dedicated care of a provider with expertise in the unique needs of older patients. Patients are invited to join the program by their current Reliant primary care team.

How does the program work?

Patients who are invited to join the Geriatric Primary Care Program will move from their current Reliant primary care provider, where they are seen traditionally in the office, to a Geriatric care team. Their new PCP will be an Advanced Practitioner who works alongside a Geriatric Physician, and other team members. Medical visits will occur where it is most convenient for patients – at home.

  • Proactive, well-coordinated care that can help reduce the need for ER visits or inpatient admissions
