Improving Massachusetts Partnering and Care Transitions- IMPACT 2.0

We believe that real-time messaging will make some ER transports, expensive tests, and hospital admissions unnecessary; as well as help ensure follow-ups and/or increased home services in order to reduce subsequent ER visits and readmissions.  Sending office visit notes to Home Health will improve shared-care coordination, keeping them up to date with changes in treatment plans and to improve patient compliance with those plans.  Automated query of the MA PMP will reduce inappropriate opiate prescriptions.  With the lessons learned from IMPACT 2.0, the state can move forward with enhancing the MA HIway RLS informed by objective evidence of its value.
