Reliant Receives Three Distinguished Awards

Reliant Medical Group has recently received three distinguished awards from the American Heart Association (AHA) and the American Medical Association (AMA).

First, Gold-level recognition from the AHA’s 2020 Check. Change. Control. Cholesterol program. This important program recognizes healthcare systems for improving the quality of care through awareness, detection, and management of high cholesterol.

We have also received Gold-level recognition as part of the 2020 Target: BP Recognition Program for our commitment to helping improve blood pressure control rates among our patients. Gold-level recognition signifies that 70% or more of our adult patients have their blood pressure controlled.

Last but certainly not least, we have received the Type 2 Diabetes Gold Award for implementing quality improvement measures addressing the relationship between type 2 diabetes and cardiovascular disease in outpatient settings. This award recognizes practices that have fulfilled specific criteria and met defined thresholds for clinical quality measures related to diabetes control and cardiovascular risk factors.

All of the recognition noted above reflects a tremendous amount of hard work and dedication to our patients by our providers and clinic support staff.
